Baseball registration for boys and girls ages 5 to 18, who reside on the east side of the river, south of College Drive and east of Preston Ave., will be held Thursday, March 4, 7:00 to 9:00 p.m. and Saturday, March 6, 2:00 to 4:00 p.m. at St. Luke Elementary School, 275 Emmeline Road.

Please print and complete two copies of this form and bring them to in-person registration.  One of the completed forms will be used as your receipt for the fitness tax credit.

Pee Wee, Bantam, and Midget players are responsible for supplying their own baseball pants and helmets.  Please see the Braves apparel descriptions and price list.

Rookie: 2003 or 2004	Jr. Mosquito: 2001 or 2002	Mosquito: 1999 or 2000
